Vaccines are available at JPS locations by appointment only. Eligible patients and family members may schedule an appointment to receive the COVID-19 vaccine in one of the following ways:
- Direct schedule using MyChart
- Call 817-702-1100, option 7 to schedule an appointment

To ensure the safety of our patients, JPS requests one of the following documents be presented prior to receiving the vaccine:
- Driver's license
- State issued identification card
- Report card that includes dates of birth
- Birth certificate
- Vaccine record that includes date of birth
If you are unable to make an appointment at JPS because of eligibility, vaccine supply, or appointment availability, you may register with Tarrant County Public Health and make a vaccine appointment with a local hub provider. Find a list of available hub providers in your area.
Do you need a ride to a JPS clinic to get your COVID-19 shot? JPS is teaming with Trinity Metro and Tarrant County so that JPS patients can get free rides to and from COVID-19 vaccination appointments. Click here for more information.
